CLAIM DOCUMENTS

- The original insurance policy

In case of urgent medical help;

- The complete medical documentation confirming the emergency treatment;

- Diagnostic report, the description of the treatment and of the patient health condition during the stay in the hospital and the final hospital report;

- The original invoice for the medical services offered from the foreigner organs even the expenses of the treatment.

- The documentation from the attendant and/or hospital organ for diseases found and treated before the journey;

- Request for indemnification according to the determined form from the Insurer completed from the Insured, or the inheritor, or the beneficiary of the insurance.

In case of death from accidents or acute disease, not existing before:

- official notification or report given by the foreigner authorities for the accident (the circumstances);

- legal-medical report for the death and the report of the autopsy;

- death certificate;

- valid heritage decision.
